Eligibility

All  CSW students participating in a DIAA  sport at CSW must have a Delaware Interscholastic Athletic Association Physical Examination Form on file in the school nurse’s office prior to participating in any practices, competitions, or athletic events. Delaware regulations require that all sports physicals must be performed after APRIL 1 of the current school year. All sports physicals must be completed using the approved Delaware Interscholastic Athletic Association form only. Competing, practicing, or scrimmaging without a current DIAA sports physical on file is not only a violation of the DIAA regulations but also places our school in a tenuous legal position. Most importantly, participating without a valid sport physical may place your child at risk if not medically cleared.

All sports physicals are due at least two weeks before the start of each season in order to provide sufficient time for review and creation of the eligibility list. All sports physicals go to the school nurse who will review them for any health issues and completion. Once they are reviewed, page 4 will be copied for the athletic department and the original physical stays in the student medical file in the nurse’s office. STUDENTS ARE NOT ELIGIBLE TO TRY-OUT, PRACTICE, SCRIMMAGE, OR COMPETE WITHOUT AN UP TO DATE DIAA PHYSICAL ON FILE.

FALL SPORTS - DUE AUGUST 1
WINTER SPORTS - DUE NOVEMBER 1
SPRING SPORTS - DUE FEBRUARY 15 

Where To Get Your Sports Physicals Done:

There are many options for getting sports physicals done in time for due dates. Depending on your insurance reimbursement schedule and availability, here are some options:

  • Your personal family physician

  • School-Based Health Center (651-2100-open all school year, closing usually the 3rd week of June and re-opening mid-July)

Tips Concerning Physicals

  1. Do not wait until the last minute. DIAA forms are due two weeks before try-outs start, not the day of try-outs.

  2. Make a copy and keep it for your personal records

  3. Do not give sports physicals directly to coaches. They are not responsible for reviewing them. All sports physicals must go to the school nurse where they will be reviewed and copied for the athletic department.

  4. Make sure the form is completely filled out including a total of 11 signatures. The student must be cleared to participate on or after April 1 based on a physical examination conducted within 12 months of the health care provider’s signature.

  5. Any sports physical that is missing signatures, dated incorrectly, or incomplete will be returned.

Sports-Related Injuries Or Surgeries

Any athlete who has a sport/related injury or surgery must notify the school nurse and must be cleared by their treating physician before they will be allowed to participate again. All medical notes should be brought to the nurse’s office directly.

Concussions

All concussions must be reported immediately to the school nurse regardless of how the concussion was sustained. CSW follows a strict concussion policy that outlines the guidelines for each student’s athlete return to school academics and eventually athletics. The school nurse serves as the liaison to the guidance department and athletic trainer for all concussions. Any medical documentation must be given directly to the school nurse (not the athletic trainer). Students must report to the school nurse’s office in the morning of their first day of return to school after a concussion. See the actual CSW concussion policy for more detailed information.

Attention all Wrestlers and Parents of Wrestlers

At CSW, we strive to maintain a healthy and hygienic experience for our wrestlers. Please read the following tips and guidelines for hygiene for wrestlers and discuss them with your student-athlete. It is very disappointing when a wrestler has to be held from practice or competition due to a contagious skin lesion, so we are trying to be proactive and encouraging our athletes to do the same.
