Incoming Freshmen & Transfers

 

All students are expected to have the following on file in the nurse’s office a MONTH PRIOR to the start of school: 

1.  Physical examination

2.  Immunization record

3.  Tuberculosis risk assessment or skin testing documentation

4.  Emergency treatment card

PHYSICAL EXAMINATIONS:

There are two different physical exam forms.  If your child will be a sports candidate at any time during the academic school year, use the Delaware Interscholastic Athletic Association (DIAA) form.  A doctor or nurse practitioner must perform an exam and complete page 3. The form must be dated after May 1. If your child will not be participating in sports, use the Delaware School Physical Examination Form. Physicals may be performed at the Wellness Center at no cost. Call 651-2100 for an appointment.  A student may not participate in any practices, scrimmages, or try-outs unless a DIAA form is on file.

IMMUNIZATIONS:

All students entering the Charter School of Wilmington are required to comply with the immunization regulations adopted by the Delaware Department of Education. 

 Documentation of the vaccines listed below is required for attendance.  The only exceptions are as follows: 1) Your child’s physician has provided written documentation that one or more of these vaccines are contraindicated for health reasons. 2) A notarized affidavit stating immunizations are against your religion.  3) Immunity is proven by blood titre. A copy of immunizations can be provided by your child's physician or by filling out this form.

 

Delaware’s Schedule of Required Immunizations:

  • 4 or more doses of DPT (diphtheria, pertussis, tetanus)
  • 3 or more doses of  OPV (oral polio vaccine)
  • 3 doses of Hepatitis-B vaccine
  • 2 doses of MMR (measles, mumps, rubella) 

Immunizations Recommended but not required:

  • Tdap Vaccine at ages 11-18 years old (tetanus, reduced diphtheria and acellular pertussis)
  • Meningococcal Vaccine recommended for 11-12 year old or before high school entry at 14-15 years old
  • Varicella vaccine:______or date of chicken pox_______

TUBERCULOSIS SCREENING/SKIN TESTING:

The state of Delaware requires all new school enterers to show proof of tuberculosis risk assessment, and if needed, skin testing within the past 12 months.  A new enterer is anyone entering the public school system for the first time. (Out of state, private or parochial schools, home schooled, out of country) Written documentation must be provided.  Anyone who has already been in the public school system needs to provide the date and results of their last tuberculosis testing or screening but does not need to repeat the screening or testing.  It is recommended that anyone who has traveled for at least two weeks to areas where tuberculosis is endemic (Africa, India, and the Caribbean) should have skin testing.

Emergency Treatment Card:

The Emergency Treatment Form must be filled out ON A YEARLY BASIS. This information is confidential and is held in the nurse’s office.  Make sure to sign permission for medication administration.  Please notify me when there are any phone number, work, or address changes.

Medication Policy:

Students are not permitted to carry any form of medication other than inhalers and epi-pens with them at school including over the counter medication.  Please send in any medication your child will need to the nurse’s office with a written note explaining the reason for the medication and the dosages and times.  If your child is asthmatic they need to keep a back-up inhaler at school.  The medication form is available online.

The Wellness Center:

The Charter School of Wilmington is fortunate to have a Wellness Center on site.  They are open Monday through Friday.  The center is staffed by Christiana Care and employs a full-time nurse practitioner and social worker.  A physician and nutritionist are available part-time.  Physical exams are performed free of charge.  Immunizations, allergy shots, tuberculosis skin testing, and strep-throat testing are available.  A student must be registered in order to receive services.  Please call the Wellness Center at 651-2100 to schedule a physical exam.  Please call early to avoid the last minute rush for sports physicals.  The center has limited hours during the summer.
